fa تکنیک جدید برای کاهش بروز عوارض Rotating Withdrawal Technique :IUD تولیدمثل Reproduction پژوهشی اصيل Original Research <p dir="RTL" style="text-align: justify"><span style="font-size:12px;"><span style="font-family:iransharp;"><strong>اهداف:</strong> استفاده از وسایل داخل رحمی <span dir="LTR">(IUDs)</span> سال‌ها به‌عنوان یکی از موثرترین، بی‌خطرترین و اقتصادی‌ترین روش‌های جلوگیری موقت از بارداری مطرح بوده است. هر چند که عوارض مختلفی برای آن گزارش شده است. در این پژوهش تلاش شد تا با استفاده از تکنیک خاص جای‌گذاری <span dir="LTR">IUD</span> از میزان بروز عوارض آن کاسته شود. هدف این مطالعه معرفی تکنیک <span dir="LTR">Rotating withdrawal</span> برای اولین‌بار بود که به‌منظور کاهش مشکلات و عوارض حاصل از به‌کارگیری <span dir="LTR">IUD</span> بررسی شد.<span style="font-size:10.0pt;"></span><br> <strong>مواد و روش‌ها: </strong>این مطالعه کوهورت تاریخی روی ۱۱۹۹ زن که بین سال‌های ۷۴-۱۳۶۴ برای گذاشتن <span dir="LTR">IUD</span> به مرکز پزشکی صارم مراجعه کرده بودند، انجام شد. پس از معاینات اولیه با روش <span dir="LTR">Rotating withdrawal</span>، برای آنها <span dir="LTR">IUD</span> گذاشته شد. پیگیری این افراد به مدت ۲ سال انجام شد<span dir="LTR">.</span><span style="font-size:10.0pt;"></span><br> <strong>یافته‌ها: </strong>جابه‌جایی <span dir="LTR">IUD</span> تنها در (۱/۶۷%) ۲۰ مورد، آن هم در ماه اول جای‌گذاری دیده شد. برای این نمونه‌ها جای‌گذاری دوباره انجام شد که (۰/۰۳۵%) ۴ مورد دوباره پس از یک ماه دچار جابه‌جایی شدند، که به آنها روش دیگری برای پیشگیری از بارداری پیشنهاد شد. در دیگر افراد، موردی از جابه‌جایی و یا بارداری پس از ۲ سال دیده نشد<span dir="LTR">.</span></span></span><span style="font-family:iransharp;"><span style="font-size:10.0pt;"></span></span></p> <p style="text-align: justify"><span style="font-size:12px;"><span style="font-family:iransharp;"><strong><span dir="RTL">نتیجه‌گیری: </span></strong><span dir="RTL">کاربرد تکنیک Rotating withdrawal با دقت و حوصله، می‌تواند میزان بروز بارداری هنگام کاربرد IUD را به حداقل برساند.&nbsp;بنابراین علاوه‌ بر&nbsp;جنس و کیفیت </span>IUD<span dir="RTL">، روش</span> <span dir="RTL">جای‌گذاری نیز می‌تواند اثربخشی آن را افزایش دهد.</span></span></span></p> <p dir="ltr" style="text-align: justify"><strong>Aims: </strong>The use of intrauterine devices (IUDs) has been one of the most effective, the safest and cost-effective methods for preventing pregnancy, for a long time. However, some complications have been reported for IUDs. The complications may be reduced by using a specific technique for placement of IUD. The objective of this study was to introduce the rotating withdrawal technique for the first time, which was investigated to reduce the complications of IUD insertion.<br> <strong>Materials & Methods:</strong> This historical cohort study was conducted on 1199 females who referred to Sarem Medical Center for IUD insertion from 1984 to 1995. After primary examinations, IUD was inserted by rotating withdrawal technique. The subjects were followed up for 2 years.<br> <strong>Findings:</strong> Displacement was observed just in 20 cases (1.67%). All of them were seen in the first month after placement. For these cases, IUD insertion was done again. After the second replacement, displacement was observed in 4 cases (0.035%) after one month. Therefore, it was recommended to them to use another preventive method. There was no pregnancy or displacement in the other cases during 2 years.<br> <strong>Conclusion:</strong> The rotating withdrawal technique can minimize the pregnancy rate when IUD is used.
