fa گزارش جنین 27 هفته‌ای با کاردیومگالی و هیدروسفالی و بررسی آسیب‌شناسی ویروس سایتومگال (CMV) ناباروری Sterility گزارش مورد Case Report <p dir="RTL" style="text-align: justify"><span style="font-size:12px;"><span style="font-family:iransharp;"><strong>مشخصات بیمار:</strong> <em>سایتومگالوویروس</em> <span dir="LTR">(CMV)</span> یکی از اعضای خانواده بتا- هرپس‌ویروس‌هاست که تولید انکلوزیون‌های سیتومگالیک در سلول‌های آلوده می‌کند<span dir="LTR">.</span> این ویروس، سندرومی شبیه به مونونوکلئوز عفونی تولید می‌نماید<span dir="LTR">.</span> در سرم بسیاری از افراد آنتی‌بادی اختصاصی علیه این ویروس، حاکی از برخورد افراد در طول زندگی با این ویروس است<span dir="LTR">.</span> اولین برخورد با این ویروس در دوران بارداری می‌تواند باعث ناهنجاری‌ها از جمله کاردیومگالی، هیدروسفالی و حتی مرگ جنین شود<span dir="LTR">.</span> این گزارش موردی مربوط به یک جنین هفته‌ای از مادر ساله در بیمارستان صارم است. بنابر تشخیص‌های پاتولوژیک حاصل از نمونه‌های رنگ‌آمیزی‌شده با روش <span dir="LTR">H&E</span> و <span dir="LTR">PAS</span>، آزمون‌های مولکولی برای تشخیص ویروس سایتومگال انجام شد و پس از استخراج <span dir="LTR">DNA</span> از بلوک‌های پارافینی حاصل از بافت‌های مختلف جنین به‌روش <span dir="LTR">Nested-PCR</span> با کمک دو جفت پرایمر (<span dir="LTR">CMTR۱</span>، <span dir="LTR">CMTR۲</span>، <span dir="LTR">CMTR۳</span> و <span dir="LTR">CMTR۴</span>) در دو مرحله، حضور این ویروس بررسی شد.<br> <strong>نتیجه‌گیری: </strong>با توجه به کاریوتایپ سالم و نداشتن مشکلات ژنتیکی و بررسی‌های مولکولی، علت اصلی تغییرات حاصل در جنین از جمله کاردیومگالی و ‌هیدروسفالی و ‌همچنین علت مرگ در هفتگی جنین، وجود <em>سایتومگالوویروس</em> است. </span></span><br> &nbsp;</p> <p dir="ltr" style="text-align: justify"><strong>Introduction:</strong> <em>Cytomegalovirus</em> (CMV) is a member of the &beta;-Herpes family, which produces cytomegalic inclusions in infected cells. This virus produces a syndrome similar to infectious mononucleosis. Presence of specific antibodies against <em>Cytomegalovirus</em> in persons &lsquo; serum shows that, people contact with CMV during their lifetime. The first encounter with this virus during pregnancy can cause abnormalities such as cardiomegaly, hydrocephalus, and even fetal death.<br> <strong>Patient Information:</strong> This case report concerns a 27 weeks fetus from a 27 year old mother at Sarem Hospital. According to the pathologic findings of the samples stained with H&E and PAS methods, molecular tests were performed to detect <em>Cytomegalovirus</em>, and after DNA extraction from paraffin blocks obtained from different tissues of the fetus using Nested-PCR with two pairs of primers (CMTR1, CMTR2, CMTR3, and CMTR4), the presence of this virus was investigated in two stages.<br> <strong>Conclusion:</strong> Regarding the healthy karyotype and the absence of genetic problems and molecular investigations, the main cause of the changes in the fetus, including cardiomegaly, hydrocephalus, and the cause of fetal death in the 27th week, is the presence of <em>Cytomegalovirus</em>.</p> سایتومگالوویروس, مرگ جنین, کاردیومگالی, هیدروسفالی Cytomegalovirus, Fetal Death, Cardiomegaly, Hydrocephalus 35 38 http://saremjrm.com/browse.php?a_code=A-10-1-8&slc_lang=fa&sid=1 M.
