Introduction: Laparoscopic vaginoplasty applying Modified Vecchietti technique is a novel method for vaginal agenesis treatment. In this method, a combination of dilation and surgery without grafting is applied. This technique was invented in 1992 and was used at Sarem hospital in Iran for the first time in 2009.

Case presentation: Nine women between ages 17-35 years with the diagnosis of Mayer- Rokitansky-Kuster-Hauser Syndrome were investigated by laparoscopic vaginoplasty using modified mecchietti between 2009 and 2014. Only two out of nine patients, had complaints about vaginal length because of lack of regular intercourse and vaginal generator. The average time for surgery was 2 hours and 8 minutes and the mean vaginal length was 6.5 cm. According to the last follow up, minimum vaginal length was 3 cm and maximum vaginal length was measured as 10cm. Also, mean staying time at hospital was 7.3 days.

Conclusion: In total, the mentioned technique, in comparison with other vaginoplasty methods has less side effects and at the same time is as effective as the other techniques. Because of lack of use of transplantation, there was no side effect due to transplantation.
